WASHINGTON — Pools of water on the floor and old, hard-to-clean equipment at a Colorado farm's cantaloupe-packing facility were probably to blame for the deadliest outbreak of foodborne illness in 25 years, the Food and Drug Administration said.
SALINAS, Calif. - Foods, LLC of Salinas, Calif., which produces bagged salads for Hy-Vee and other supermarket chains, has issued a voluntary recall of 2,154 cases of various bagged salads due to possible contamination with Listeria monocytogenes.

The Minnesota Department of Agriculture laboratory has confirmed E. coli O157:H7 contamination of in-shell hazelnuts (also known as filberts) collected from the home of an ill Minnesotan.
